The White Box is a fun little retro-clone of the Original 74 version of D&D.
The paperback versions are cheap, and the PDF is available for free.
There are 3 existing Character Sheets for White Box, I’ve cleaned them up a bit and made them fillable.
The first two also have a location for a portrait or image. If you click on those areas, you should be able to import an image into the sheet.
